  2. Complete the Nutritional History Questionnaire below

  3. Email, fax, or mail the completed Questionnaire

  4. The next business day, Veterinary Nutrition Care will contact you to arrange your visit

  5. Arrive (with your pet) for your scheduled visit. You'll be happy that the preparation steps helped make the visit worthwhile. 

Veterinary Nutrition Care's Vet Consults put advanced nutrition in your vet's hands. 


  1. ​Make an appointment to discuss your pet's nutrition with your vet.

  2. Your vet requests a Vet Consult

  3. Read "what to expect." Really. Especially about preparation and fees

  4. Complete the Nutritional History Questionnaire below

  5. Email, fax, or mail the completed Questionnaire

  6. Your veterinary nutritionist will complete your pet's nutritional assessment and nutrition plan; your vet will receive a summary

  7.  Your vet will review the Vet Consult summary with you​
